A Liquid BBL is a non-surgical injectable treatment intended to create a moderate change in buttock or hip contour without liposuction or fat transfer. At InjectCo The Woodlands, Misty and Lindsey may use collagen-stimulating injectables such as Sculptra or Radiesse for this purpose. Buttock and hip use is off-label: these products are not FDA approved for buttock augmentation, and the FDA recommends against injectable fillers for buttock body contouring.
Sculptra and Radiesse are FDA-approved injectable products for specific aesthetic indications, but neither product is FDA approved for augmentation of the buttocks or hips.
The FDA says no dermal filler or other injectable filler is approved to make the buttocks larger, and its dermal filler guidance recommends against using injectable fillers for body contouring or enhancement of the buttocks.
Using these products in a Liquid BBL is therefore outside their FDA-approved indications. The FDA has not reviewed or endorsed this treatment for buttock or hip augmentation.

Are you trying to change your shape, or dramatically increase your size?
When someone asks us about a Liquid BBL, one of the first things we need to understand is whether they are asking for a different shape or a major increase in size. Those are not the same goal. Many people use “BBL” to mean “bigger,” but an injectable is better understood as a moderate contour treatment.
Injectable treatment can sometimes make sense for a patient who wants a moderate contour change, smoother hip dips or better balance from one side to the other. It is not a substitute for surgical fat transfer when the goal is a large change in projection or overall size.
We look at the patient's existing projection, hip contour, symmetry, tissue quality, weight stability and the result they are showing us before we talk about treatment. Sometimes the most useful part of the consultation is explaining that the picture they brought in cannot realistically be created with an injectable. We would rather tell someone that before they spend money than build a treatment plan around an outcome the procedure cannot deliver.
When a patient is an appropriate candidate, the goal is proportion and contour, not simply putting in the largest amount of product possible.

Sculptra is an injectable made of poly-L-lactic acid (PLLA) microparticles. It is supplied as a powder and mixed with sterile water before use, so it goes in as a liquid suspension. Much of the early fullness is that liquid, which the body absorbs. Over the following months, PLLA prompts a tissue response associated with new collagen formation. That is why Sculptra behaves differently from a hyaluronic acid filler, which adds volume right away.
Radiesse is an injectable implant made of calcium hydroxylapatite particles suspended in a gel. The gel gives some fullness sooner, and further change reflects the tissue's response to the particles over time. Hyperdiluted Radiesse is Radiesse mixed with additional liquid so it spreads in a thinner layer, and it is sometimes chosen for skin quality rather than volume. How a product is prepared and used in a hyperdiluted treatment may differ from its FDA-approved labeling.
Neither product can simply be dissolved the way a hyaluronic acid filler can with hyaluronidase. The FDA notes that fillers not made of hyaluronic acid may be difficult or impossible to remove, which is one more reason we plan conservatively.
Sculptra's approved uses. Its current FDA labeling lists three uses. The first is correcting shallow to deep nasolabial folds and other facial wrinkles. The second, added in April 2023, is correcting fine lines and wrinkles in the cheeks. The third is restoring facial fat loss in people with HIV.
Radiesse's approved uses. Radiesse is a calcium hydroxylapatite filler. The FDA describes calcium hydroxylapatite fillers as approved for wrinkle filling in the face and for the back of the hand.
What is not approved. Neither product is FDA approved for augmentation of the buttocks or hips. The FDA's dermal filler guidance lists increasing the size of the buttocks among the uses it recommends against and states that these uses are not approved.
Clinicians can sometimes use a device outside its labeled indications. Here, though, the FDA has gone further than simply not approving the use: it recommends against it. We think you should hear that clearly before you decide.
Why facial data does not transfer. The FDA's summary of the 2023 cheek approval describes results lasting up to 24 months, and Sculptra's facial labeling reports follow-up to 25 months after the last injection. Those studies were in faces. They are not evidence for how long a Liquid BBL lasts or how often complications occur in the buttocks, and we do not present them that way.
Surgical gluteal fat grafting, the procedure behind a surgical Brazilian butt lift, transfers a patient's own fat and can produce a larger change than injectable treatment. Major plastic surgery societies have repeatedly raised safety concerns about gluteal fat grafting, including fatal fat embolism. Their joint safety statement of August 18, 2022 supports measures such as placing fat only in the subcutaneous layer and using real-time ultrasound guidance. It was issued by the American Society of Plastic Surgeons, The Plastic Surgery Foundation, The Aesthetic Society and the Aesthetic Surgery Education and Research Foundation.
Earlier task-force publications reported very high mortality estimates, including figures around 1 in 3,000. These are historical estimates and should not be read as a current mortality rate for surgical BBL.

Neither option is right for everyone. If the goal is a dramatic change in size, discuss surgical options with a board-certified plastic surgeon. InjectCo does not perform surgery, and we will tell you plainly if a Liquid BBL will not get you where you want to go.

What Sculptra's labeling excludes. The current FDA labeling rules Sculptra out in four situations:

Pregnancy and breastfeeding. These are not listed as formal contraindications. Sculptra's labeling states that its safety and effectiveness have not been evaluated in pregnant or breastfeeding patients, so tell your nurse if either applies and the timing of any treatment will be decided with you.
Some goals need actual fat transfer, meaning a large change in projection or overall size. When that is your goal, we will tell you an injectable is the wrong tool and suggest you talk with a board-certified plastic surgeon about a surgical Brazilian butt lift. We would rather tell you that before you spend money.
We also recommend waiting when your weight is still changing, when there is an infection or inflammation in the area, or when your medical history needs more review first. A consultation is not a commitment to treatment.

Because using Sculptra or Radiesse in the buttocks and hips is off-label, complication percentages from FDA facial studies should not be presented as established Liquid BBL complication rates. We do not quote them that way.
Injectable procedures can carry vascular risk if material unintentionally affects a blood vessel. Product labeling and the FDA's filler guidance describe potentially serious vascular complications with injectable fillers. If you develop severe or unusual pain, concerning skin-color changes, neurologic symptoms or another rapidly developing symptom after treatment, contact the clinic promptly and seek emergency medical care when symptoms are severe.
Radiesse is visible on X-rays and may obscure underlying features, so tell any imaging team that you have been treated.
The FDA has not approved injectable silicone for any aesthetic procedure, including body contouring, and it warns that silicone injections can lead to long-term pain, infection, permanent disfigurement, embolism, stroke and death. Walk away from anyone offering butt shots of silicone, hydrogel or an unnamed product, injections in a home, hotel or party setting, or treatment by someone who is not a licensed medical professional working under physician oversight. Ask which product will be used and how it was sourced. A legitimate clinic will tell you.

Contact InjectCo if swelling, redness, tenderness, firmness, asymmetry or another symptom is worsening rather than improving, or if you develop a new lump or nodule.
The clinical team determines whether the concern fits expected recovery, needs an in-person assessment, or requires urgent medical evaluation. Seek emergency care right away for severe or rapidly worsening symptoms.
Because Sculptra and Radiesse cannot simply be dissolved like a hyaluronic acid filler, unexpected results or reactions require clinical assessment rather than an automatic “dissolve and restart” approach.
At follow-up, your nurse compares your progress with the baseline photos taken at your assessment.

A surgical Brazilian butt lift moves your own fat under anesthesia and can create a larger change; its best-known serious risk is fat embolism. A Liquid BBL injects Sculptra or Radiesse under numbing cream for a moderate, gradual change in shape, and its risks are different, including nodules, infection and vascular complications.
Sculptra and Radiesse are the two products used for a Liquid BBL at our Woodlands clinic. Sculptra (poly-L-lactic acid) works gradually by prompting your body's own collagen response. Radiesse (calcium hydroxylapatite) gives some fullness sooner, then works through a similar tissue response over time. Neither is FDA approved for the buttocks, so both are used off label. Sculptra's labeling recommends massaging the treated area for 5 minutes, 5 times a day, for 5 days after its FDA-reviewed facial treatment. InjectCo may use a similar protocol after a Liquid BBL. It is intended to help distribute the product and does not guarantee that nodules will not form. Follow your nurse's specific instructions.
