Polylactic Acid (PLA)

PLA works differently than HA. Rather than adding direct volume, it stimulates fibroblast proliferation and collagen production, creating enhancement through induced tissue growth. This biostimulatory mechanism produces gradual results over weeks to months.

Studies show PLA provides effective girth enhancement with satisfaction levels comparable to HA at certain timepoints. However, the same comparative study found local adverse event rates of 11.9% for PLA versus 7.2% for HA.

More concerning, PLA has been associated with granuloma formation, where the body creates inflammatory nodules around filler particles. Case reports document granulomas appearing months after PLA injection, sometimes requiring surgical excision when conservative treatment fails.

PLA cannot be dissolved like HA. Once injected, your only options for managing complications are waiting for natural degradation, steroid injections, or surgical removal.

Polymethylmethacrylate (PMMA)

PMMA, sold under brand names like Bellafill, contains microscopic plastic spheres suspended in collagen carrier gel. The microspheres remain permanently in tissue, making PMMA the only truly permanent injectable filler option.

This permanence comes with significant tradeoffs. The same comparative study found PMMA patients experienced the highest rate of nodules and deformities at 7.2%. More importantly, patient satisfaction was lowest in the PMMA group.

Why would permanent results produce lower satisfaction? Several factors contribute. PMMA feels harder than other fillers, creating an unnatural texture. The permanence itself causes anxiety since there’s no reversing unsatisfactory results. And the propensity for granuloma formation means complications may appear years after injection.

Research confirms that PMMA, PLA, and silicone are all associated with greater reactivity potential than HA injection. Permanent fillers create permanent complications when things go wrong.

Fat Transfer

Autologous fat transfer uses your own fat harvested through liposuction, processed, and reinjected into the penis. The appeal is using your own tissue rather than synthetic materials.

Reality disappoints most patients. The body reabsorbs 30% to 50% of transferred fat within the first year. Results are unpredictable since reabsorption occurs unevenly, often creating lumpy, asymmetric outcomes. Hard nodules of necrotic fat may form where transferred cells didn’t survive.

Fat also has soft texture unlike penile tissue, potentially creating an unnatural feel. Multiple procedures are typically needed to achieve and maintain desired volume, pushing total costs well above initial estimates.

Why HA Wins: The Critical Advantages

Several specific properties make hyaluronic acid superior for penile enhancement.

Reversibility

This advantage cannot be overstated. Hyaluronic acid can be completely dissolved using hyaluronidase enzyme. If complications occur, if you dislike results, or if you simply change your mind, reversal is possible within days.

Low doses of hyaluronidase, typically 10 to 30 units, effectively break down crosslinked HA. The effects become visible within three or more days. This safety net exists only for HA. Every other filler type commits you to permanent or semi-permanent results with no reversal option.

For fat transfer, reabsorption of 30% to 50% of transferred volume is expected, results are unpredictable with lumpy outcomes, necrotic fat nodules may form, and multiple procedures are typically required.

The pattern is clear. HA provides the best safety profile combined with the unique advantage of reversibility.

What About Longevity?

Some men worry that HA’s temporary nature is a disadvantage. After all, PMMA is permanent. Why not choose permanent results?

Consider what temporary actually means. HA results typically last 12 to 24 months. Some patients report retention for 3 to 6 years with proper protocols. Maintenance treatments every 12 to 18 months can sustain results indefinitely.

The temporary nature is actually a feature, not a bug. It means your results can evolve with your preferences. It means complications don’t become permanent problems. It means you maintain control over your body rather than committing to irreversible alteration.

If you later decide you want permanent enhancement after experiencing results with HA, that option remains available. But starting with permanent filler eliminates the option to start over.
