Non-Surgical Brazilian Butt Lift (Liquid BBL) in The Woodlands, TX

A Liquid BBL is the non-surgical alternative to a surgical Brazilian butt lift. It uses injectables to refine buttock and hip contour, making it a non-surgical butt augmentation option built around shape. It suits people who want a better shape, not a big jump in size. It is also an off-label use that the FDA recommends against, and Misty and Lindsey explain exactly what that means before you decide.

What Is a Liquid BBL?

A Liquid BBL is a non-surgical injectable treatment intended to create a moderate change in buttock or hip contour without liposuction or fat transfer. At InjectCo The Woodlands, Misty and Lindsey use one collagen-stimulating injectable for this, hyperdilute Radiesse. Buttock and hip use is off-label: Radiesse is not FDA approved for buttock augmentation, and the FDA recommends against injectable fillers for buttock body contouring.

IMPORTANT: LIQUID BBL IS NOT AN FDA-APPROVED BUTTOCK AUGMENTATION

Radiesse is an FDA-approved injectable for specific aesthetic indications, but it is not FDA approved for augmentation of the buttocks or hips.

The FDA says no dermal filler or other injectable filler is approved to make the buttocks larger, and its dermal filler guidance recommends against using injectable fillers for body contouring or enhancement of the buttocks.

Using Radiesse in a Liquid BBL is therefore outside its FDA-approved indications. The FDA has not reviewed or endorsed this treatment for buttock or hip augmentation.

Your InjectCo clinician explains the off-label nature of treatment, its limitations, the alternatives and the relevant risks before you decide whether to proceed.

Shape Before Size: How We Decide Whether a Liquid BBL Makes Sense

Clinical insight from Misty, MSN, RN, and Lindsey, MSN, APRN, CPNP-AC, InjectCo The Woodlands.

Are you trying to change your shape, or dramatically increase your size?

When someone asks us about a Liquid BBL, one of the first things we need to understand is whether they are asking for a different shape or a major increase in size. Those are not the same goal. Many people use “BBL” to mean “bigger,” but an injectable is better understood as a moderate contour treatment.

Injectable treatment can sometimes make sense for a patient who wants a moderate contour change, smoother hip dips or better balance from one side to the other. It is not a substitute for surgical fat transfer when the goal is a large change in projection or overall size.

We look at the patient's existing projection, hip contour, symmetry, tissue quality, weight stability and the result they are showing us before we talk about treatment. Sometimes the most useful part of the consultation is explaining that the picture they brought in cannot realistically be created with an injectable. We would rather tell someone that before they spend money than build a treatment plan around an outcome the procedure cannot deliver.

When a patient is an appropriate candidate, the goal is proportion and contour, not simply putting in the largest amount of product possible.

What Misty and Lindsey Assess at Your Consultation

Misty, MSN, RN and Lindsey, MSN, APRN, CPNP-AC are the Clinical Aesthetic Nurse Injectors at InjectCo MedSpa The Woodlands, and both perform Liquid BBL. Misty spent 18 years in neonatal intensive care, including as Neonatal ICU Nurse Manager at Texas Children's Hospital The Woodlands. She holds a master's degree in Nursing Leadership and Management and trains new registered nurse injectors for InjectCo. Lindsey is an Advanced Practice Registered Nurse with a Master of Science in Nursing and more than 16 years of direct patient care. Her CPNP-AC is a pediatric acute care nurse practitioner credential, not an aesthetic-injector certification.

What they look at before recommending treatment

  • How much projection you already have
  • Hip contour, including hip dips
  • Differences between your left and right sides
  • How your waist, hips and buttocks transition into each other
  • Skin and tissue quality
  • Whether your weight is stable
  • The result you are hoping for, including any photos you bring

How nurse-led treatment works in Texas

Texas treats cosmetic injections as medical acts that a physician may delegate under the Texas Medical Board's delegation rules (22 Texas Administrative Code, Chapter 169). Those rules require:

  • training in the procedure, its contraindications and complication management
  • a signed written protocol
  • an established patient relationship and medical record
  • telling you who will perform the treatment
  • a person trained in basic life support on site
  • a physician or advanced practice provider on site or immediately available for emergencies

At InjectCo, treatment is delegated under the physician oversight of Dr. Josh Allen, D.O., Medical Director. Learn more from the Texas Medical Board, and verify a nurse's license through Nursys.

Radiesse Butt Lift: What the Product Does

Radiesse is a biostimulator: an injectable that gives some support and then prompts a gradual tissue response instead of only filling space.

It is an injectable implant made of calcium hydroxylapatite particles suspended in a gel. For a Liquid BBL it is hyperdiluted, meaning mixed with additional liquid so it spreads in a thinner layer over a larger area. The gel gives some lift on the day, and the tissue's response to the particles builds volume and collagen for about six months. How a product is prepared and used in a hyperdiluted treatment may differ from its FDA-approved labeling.

Radiesse cannot simply be dissolved the way a hyaluronic acid filler can with hyaluronidase. The FDA notes that fillers not made of hyaluronic acid may be difficult or impossible to remove, which is one more reason we plan conservatively.

Regulatory status

FDA-Approved Uses vs This Off-Label Buttock Use

An FDA-approved product is not the same thing as an FDA-approved use.

Radiesse's approved uses. Radiesse is a calcium hydroxylapatite filler. Its FDA labeling covers moderate to severe facial wrinkles and folds, facial fat loss in people with HIV, volume loss in the back of the hands and, diluted, décolleté wrinkles in adults 22 and older.

What is not approved. Radiesse is not FDA approved for augmentation of the buttocks or hips. The FDA's dermal filler guidance lists increasing the size of the buttocks among the uses it recommends against and states that these uses are not approved.

Clinicians can sometimes use a device outside its labeled indications. Here, though, the FDA has gone further than simply not approving the use: it recommends against it. We think you should hear that clearly before you decide.

Why facial data does not transfer. Radiesse's approval studies were in the face, hands and décolleté. They are not evidence for how long a Liquid BBL lasts or how often complications occur in the buttocks, and we do not present them that way. Published studies show results lasting up to about 2 years; many of our patients keep their results 2 to 4 years, depending on metabolism and lifestyle.

Surgical or non-surgical

Surgical BBL vs Liquid BBL: Which One Matches Your Goal?

Surgery changes size. Injectables refine shape. The risks and recovery differ too.

Surgical gluteal fat grafting, the procedure behind a surgical Brazilian butt lift, transfers a patient's own fat and can produce a larger change than injectable treatment. Major plastic surgery societies have repeatedly raised safety concerns about gluteal fat grafting, including fatal fat embolism. Their joint safety statement of August 18, 2022 supports measures such as placing fat only in the subcutaneous layer and using real-time ultrasound guidance. It was issued by the American Society of Plastic Surgeons, The Plastic Surgery Foundation, The Aesthetic Society and the Aesthetic Surgery Education and Research Foundation.

Earlier task-force publications reported very high mortality estimates, including figures around 1 in 3,000. These are historical estimates and should not be read as a current mortality rate for surgical BBL.

From our nurse injectors

Why the First-Day Fullness Is Not Your Final Result

With Radiesse, we do not judge success by how the area looks immediately after the appointment. You will see some lift from the gel, along with normal swelling from the injections, and that early appearance changes. We talk about the result in terms of months rather than the first few days because the collagen response keeps building for about six months. That is why we do not recommend deciding right away whether you “need more” based on how the area looks after treatment.
Right after treatment

Some lift and possible swelling. Not the final shape.

First 5 days

Swelling settles and you massage after a warm shower.

Up to six months

Volume and collagen keep building, at a pace that varies by person.

Follow-up

Your nurse compares progress with your baseline photos before discussing anything further.

These are approximate follow-up expectations, not guaranteed outcomes or fixed dates. Clinical insight from Misty, MSN, RN, and Lindsey, MSN, APRN, CPNP-AC.
Is this right for you

Who May Be a Candidate for a Liquid BBL in The Woodlands?

A Liquid BBL may suit you if you want to change your shape rather than your size.

It may be a fit if you:

  • Want smoother hip dips or a softer transition from waist to hip
  • Want better balance between the left and right sides
  • Want a moderate lift in contour, not a large increase in size
  • Are at or near a stable weight
  • Are comfortable with a result that develops over months

It is not a fit if you:

  • Want a dramatic increase in size or projection
  • Are actively losing or gaining significant weight
  • Have an infection or inflammation where the product would be placed

What Radiesse's labeling excludes. The FDA labeling rules Radiesse out for:

  • an allergy to one of its ingredients
  • severe allergies, meaning a history of anaphylaxis or several severe allergies
  • a bleeding disorder
  • an allergy to lidocaine or similar numbing medicines, for Radiesse (+) or when lidocaine is mixed in

The label adds that it has not been studied in people who tend to form keloids or hypertrophic scars, or established in autoimmune disease, so tell your nurse about either.

Pregnancy and breastfeeding. These are not listed as formal contraindications. Radiesse's safety in pregnancy and breastfeeding has not been established, so tell your nurse if either applies and the timing of any treatment will be decided with you.

Honest assessment

When We Recommend Surgery Instead, or Waiting

Sometimes the most useful thing we can do is point you elsewhere.

Some goals need actual fat transfer, meaning a large change in projection or overall size. When that is your goal, we will tell you an injectable is the wrong tool and suggest you talk with a plastic surgeon about a surgical Brazilian butt lift. We would rather tell you that before you spend money.

We also recommend waiting when your weight is still changing, when there is an infection or inflammation in the area, or when your medical history needs more review first. A consultation is not a commitment to treatment.

Clinical insight from Misty, MSN, RN, and Lindsey, MSN, APRN, CPNP-AC, InjectCo The Woodlands.

Risks and safety

What Are the Risks of a Liquid BBL?

Because this use is unapproved, we are especially open about its risks.

Possible risks of injectable treatment include:

  • swelling, bruising, tenderness and redness
  • asymmetry
  • infection
  • inflammatory reactions
  • lumps, papules or nodules
  • product ending up somewhere other than intended
  • vascular complications

Because using Radiesse in the buttocks and hips is off-label, complication percentages from FDA facial studies should not be presented as established Liquid BBL complication rates. We do not quote them that way.

Vascular risk

Injectable procedures can carry vascular risk if material unintentionally affects a blood vessel. Product labeling and the FDA's filler guidance describe potentially serious vascular complications with injectable fillers. If you develop severe or unusual pain, concerning skin-color changes, neurologic symptoms or another rapidly developing symptom after treatment, contact the clinic promptly and seek emergency medical care when symptoms are severe.

Radiesse is visible on X-rays and may obscure underlying features, so tell any imaging team that you have been treated.

Red Flag: Illegal Butt Injections

The FDA has not approved injectable silicone for any aesthetic procedure, including body contouring, and it warns that silicone injections can lead to long-term pain, infection, permanent disfigurement, embolism, stroke and death. Walk away from anyone offering butt shots of silicone, hydrogel or an unnamed product, injections in a home, hotel or party setting, or treatment by someone who is not a licensed medical professional working under physician oversight. Ask which product will be used and how it was sourced. A legitimate clinic will tell you.

Step by step

Your Appointment and InjectCo Aftercare

Everything happens in a private room with a licensed nurse, under topical numbing only. Here is the sequence.
1
Assessment

Consultation and mapping

Your nurse studies your proportions, marks the areas to treat (including hip dips or uneven contour), takes baseline photos and estimates how many sessions the plan will need.
2
20 to 30 Minutes

Numbing cream goes on

After the skin is cleaned, numbing cream sits for about 20 to 30 minutes. Your nurse explains the other comfort measures used during the session.
3
Precise

Product goes in a little at a time

Small, measured amounts are spread across the treatment zone rather than concentrated in one spot, so the shape stays balanced while the tissue response develops.
4
About 45 to 60 Minutes Total

Leaving the clinic

Plan on 45 to 60 minutes in total, numbing included. You leave the same day with no surgical recovery, only the temporary aftercare below.

InjectCo aftercare instructions

  • Massage: for 5 days after treatment, massage the area for 5 to 10 minutes after a warm shower. It helps the product spread evenly. Follow the specific instructions your nurse gives you.
  • Avoid strenuous exercise for 24 hours.
  • Most patients do not need a surgical-style recovery, but these temporary restrictions apply.
After treatment

What Happens If You Have a Concern After Treatment?

Call (281) 326-9985 if something is getting worse instead of better.

Contact InjectCo if swelling, redness, tenderness, firmness, asymmetry or another symptom is worsening rather than improving, or if you develop a new lump or nodule.

The clinical team determines whether the concern fits expected recovery, needs an in-person assessment, or requires urgent medical evaluation. Seek emergency care right away for severe or rapidly worsening symptoms.

Because Radiesse cannot simply be dissolved like a hyaluronic acid filler, unexpected results or reactions require clinical assessment rather than an automatic “dissolve and restart” approach.

At follow-up, your nurse compares your progress with the baseline photos taken at your assessment.

Liquid BBL Questions We Hear in The Woodlands

A non-surgical injectable treatment intended to create a moderate change in buttock or hip contour without liposuction or fat transfer. At our clinic it uses hyperdilute Radiesse. It is an off-label use: Radiesse is not FDA approved for buttock augmentation.

No. Radiesse is FDA approved for specific uses in the face, the back of the hands and the décolleté, but not for the buttocks or hips. The FDA recommends against using injectable fillers to increase buttock size, so a Liquid BBL is off-label, and the FDA has neither reviewed nor endorsed it for the buttocks.

The FDA's dermal filler guidance lists increasing the size of the buttocks among the uses it recommends against and says these uses are not approved. It explains that filler approvals are based on controlled studies in the face and hands, and that the risks of unapproved uses are not known. Separately, it warns that injectable silicone, which is not approved for any aesthetic use, can cause serious injury and death.

No. A Liquid BBL at our Woodlands clinic uses hyperdilute Radiesse only. The FDA warns against using injectable silicone or any filler for large-volume body contouring, and unapproved products sold as "butt shots" have been linked to serious injuries. If anyone offers you butt shots without naming the product, ask exactly what is being injected before you agree to anything.

A surgical Brazilian butt lift moves your own fat under anesthesia and can create a larger change; its best-known serious risk is fat embolism. A Liquid BBL injects diluted Radiesse under numbing cream for a moderate change in shape that builds over about six months, and its risks are different, including nodules, infection and vascular complications.

Yes. Radiesse (calcium hydroxylapatite) is the one product used for a Liquid BBL at our Woodlands clinic. It gives some lift on the day, then builds volume and collagen for about six months. It is not FDA approved for the buttocks, so it is used off label. Misty or Lindsey decides the amount and placement after assessing your shape, skin and goals at your consultation.

Shape. It can suit smoother hip dips, better side-to-side balance or a moderate change in contour. If you want a dramatic increase in size, surgical fat transfer with a plastic surgeon is the more realistic route.

It starts at $5,999. Before anything is booked, your nurse confirms what that price covers, whether further treatment is recommended and your expected total. Financing through CareCredit or Cherry may be available to approved applicants.

Radiesse gives some lift on the day, but part of what you see is swelling, and the collagen response keeps building for about six months, so we do not decide whether you need more based on how the area looks right after treatment.

About six months, with some lift from day one. The pace varies between people; your nurse reviews progress at follow-up against your baseline photos.

Published studies show results lasting up to about 2 years; many of our patients keep their results 2 to 4 years, depending on metabolism and lifestyle. Because buttock treatment is off-label, no FDA-reviewed study has measured this, so it should not be described as a proven duration.

For 5 days, massage the treated area for 5 to 10 minutes after a warm shower. It helps the product spread evenly but does not guarantee that nodules will not form. Follow your nurse's specific instructions.

Call the clinic at (281) 326-9985. The clinical team decides whether it fits expected recovery, needs an in-person assessment or requires urgent evaluation. Because Radiesse cannot be dissolved like a hyaluronic acid filler, concerns are assessed clinically. Seek emergency care for severe or rapidly worsening symptoms.

Important Safety Information

Radiesse (calcium hydroxylapatite) is FDA approved only for specific uses in the face, the back of the hands and the décolleté. It is not approved for augmentation of the buttocks or hips. The FDA recommends against using injectable fillers to increase buttock size, so a Liquid BBL is an off-label use that the FDA has not reviewed or endorsed. Possible effects include swelling, bruising, redness, tenderness, asymmetry, infection, inflammatory reactions, nodules and vascular complications. Complication rates from FDA facial studies are not established rates for this use. Radiesse labeling rules it out for ingredient allergies, severe allergies or anaphylaxis, and bleeding disorders. Radiesse cannot be dissolved with hyaluronidase, and it shows on CT scans and may show on X-rays. A Liquid BBL is not surgery and cannot match a surgical result, and outcomes differ from person to person. This page is for general education and is not medical advice.
